What to Expect on Your First Visit

What only takes you a few minutes now can save you loads of time later on. Before your first visit, we ask that you take the time to complete our brief health and dental questionnaires as well as a New Patient Form to ensure we’ve got all the information we need to give you the best care possible.

Financial Information
At Full Heart Dentistry, our mission is to give you the most comfortable and comprehensive dental care in Rochester.
On your first visit, we’ll help you make the most of your investment with an individual treatment plan and give you an estimate of your total cost.
We participate as an in-network provider with Excellus BlueCross / BlueShield. We’ll gladly process your insurance claims for you once treatment is rendered. A predetermination of treatment can also be sent to your insurance as necessary for a more accurate estimate of the amount covered by your insurance in advance.
The remaining balance is ultimately the responsibility of the patient, as your dental benefits are a contract between your employer and your insurance carrier. Your estimated copay and/or deductible for treatment is due on the day of service unless previous financial arrangements have been made.
We gladly accept most other dental insurance plans as out-of-network, and we’re happy to file claims on your behalf. Our insurance coordinator will be more than happy to answer questions on your individual insurance.
At Full Heart Dentistry, we’ll always do our best to help you to maximize your insurance benefits. However, your treatment plan is individually based on your diagnosis and is not dependent on your dental insurance benefits or lack of benefits.
Plans change. We get it. At Full Heart Dentistry, we’ll always be considerate of your time, and we ask that you be considerate of ours in return. Sometimes appointments need to be rescheduled.
We request 48 business day hours’ advance notice for rescheduling to avoid a fee.

Care Credit: We offer 6-month interest-free payment plans, and extended financing plans are also available. You can apply online, or we’ll happily assist you in our office.
